Annex 2 - Conditions consistent with the Operating Schedule
1 All customers asked to leave quietly at the end of permitted licensed hours.
2 Existing capacity limit of 150 on Fire Certificate is to remain in force.
3 Children are only allowed to attend private functions if accompanied by an adult up to 20.00 hours.
4 On all occasions Challenge 21 / Prove 18 scheme is operated staff training in place.
Any person who looks or appears to be under the age of 21 shall be asked to provide identification that they are over the age of 18. The following are the only forms of identification acceptable: -
i. UK photo driving licence
ii. Passport
iii. Proof of Age Standards Scheme Card
5 Noise from any Regulated Entertainment or Mechanical Ventilation shall be inaudible within the nearest sensitive properties or, at the discretion of the Local Authority, shall not exceed some other pre-agreed limit, which does not cause unreasonable disturbance to the residents of these properties or their guests.
6 The volume of amplified sound used in connection with any Regulated Entertainment shall, at all times, be under the control of the licensee or management and the controlling mechanism shall be operated from a part of the premises inaccessible to the public.
7 Vibration produced as a result of any Regulated Entertainment shall not be perceptible in any adjoining or nearby sensitive properties.
8 All internal lobbied doors to any entrance / exit point, fire exit doors and external windows should be closed during hours of any Regulated Entertainment except in the case of an emergency, save for the purposes of access and egress.
9 Another member of staff shall be nominated to act for the Designated Premises Supervisor, in their absence, whose identity is known by all other staff when such absence occurs.
10 A minimum of one SIA registered door supervisor to be employed at the premises every Thursday, Friday and Saturday and any Sunday preceding a bank holiday from 2100 until the premises are closed.
11 All door staff employed at the entrance / exit of the licensed premises will wear a reflective jacket / tabard of a design approved by the Lancashire Constabulary.
12 The premises are to be equipped with a closed circuit television system which meets the criteria detailed in Lancashire Constabulary's Guidance Document CCTV/AW1*, and which is well maintained and fully functional during the hours the premises are open to members of the public.

13 The local authority CCTV control room will be notified:
a. On any occasion when the premises CCTV or radio system is inoperative, and
b. When the CCTV or radio system has been brought back into use
14 The Police licensing unit shall be notified on any occasion when the CCTV or radio system is to be inoperative for a period in excess of one working day and shall provide a certificate from a competent person stating the reason for the system being inoperative and the measures which have been taken to satisfy the licence conditions.
15 CCTV recordings shall be retained for a period of 30 days and made available within a reasonable time upon request by the enforcement authorities.
16 All staff to have received suitable training in relation to the Challenge 21 proof of age scheme to be applied upon the premises. Records to evidence this will be made available to an authorised officer upon request.
17 A drugs prevention strategy for the venue shall be developed and applied. This strategy shall include arrangements for the location of posters and the distribution of other information relating to drugs risks, the dangers associated with drugs and the legal provisions relating to drug use.
18 Security arrangements are sufficient to discourage the sale and consumption of drugs and shall ensure such arrangements include regular checks of toilet areas.
19 Records of incidents involving the use and / or detection of drugs shall be maintained and those records shall be available for inspection.
20 Confiscated and found drugs shall be stored, disposed, transferred in accordance with procedures agreed with the Lancashire Constabulary.
21 The premises shall maintain a drinks pricing policy which complies with any Local Authority resolution made in accordance with approval from the Licensees Forum or any successor liaison body recognised by the Licensing Authority.
22 There shall be no drinks promotions which would contravene the British Beer and Pub Association Standards for the Management of Responsible Drinks Promotions, including Happy Hours, or the Social Responsibility for the Production and Sale of Alcoholic Drinks in the UK.
23 The Licence holder and DPS shall erect and maintain signage (in a form approved by the licensing authority) on or adjacent to all staffed exits advising patrons of the Street Drinking Order and the intention of doorstaff to enforce a 'no exit with a bottle' policy.
