1 No supply of alcohol may be made under the premises licence -
a) At a time when there is no designated premises supervisor in respect of the premises licence,
or
b) At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2 Every supply of alcohol under the premise licence must be made or authorised by a person who holds a personal licence.
3 The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol. The policy must require individuals who appear to the responsible person to be under 18 year of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Annex 2 - Conditions consistent with the Operating Schedule
1 A CCTV System is to be installed and maintained. Images to be recorded and retained for at least 28 days and to be handed to the Police / Local Authority on request.
2 Notices shall be displayed instore stating that it is an offence for under 18s to purchase alcohol. All staff are trained in relation to underage sales of alcohol. A "Think 21" proof of age scheme is in place and notices of this are prominently displayed within the store.
3 Deliveries and service vehicles (except for the delivery of newspapers and bread to be delivered at the front of the premises) shall be restricted to between 08.00hrs and 23.00hrs on Mondays to Fridays, 8.00hrs and 14.00hrs on Saturdays and not at all on Sundays and Bank Holidays.
4 The licensee shall ensure that staff arriving before 09.00 hours or departing late at night when the business has ceased trading conduct themselves in such a manner to avoid disturbances to nearby residents.
