Annex 2 - Conditions consistent with the Operating Schedule
1 The premises are to be equipped with a CCTV system, which meets the criteria detailed in Lancashire Constabulary's guidance document and which is well maintained and fully functional during the hours the premises are open to members of the public.
2 The premises shall have installed a wall-mounted screen, no smaller than 34", mounted in a prominent position within the premises, so that patrons can view live-recorded CCTV footage.
3 A staff member who is conversant with the operation of the CCTV system will be on the premises at all times that the premises is open to the public. This staff member will be able to show Police recent data or footage with the absolute minimum of delay when requested. The data or footage reproduction should be almost instantaneous.
4 The licence holder will employ sufficient door staff and shall determine an adequate number by means of a risk assessment. Not less than one door supervisor will be employed from 0300 hours to the terminal hour on Fridays and Saturdays.
5 All door staff employed at the entrance / exit of the licensed premises will wear a reflective jacket of a design approved by the Lancashire Constabulary.
6 Records of the safety of all the gas equipment in use at the premises must be on the premises and renewed annually.
7 Records of the safety and maintenance of the electrical installation for the premises must be on the premises and renewed as recommended by a competent electrician and in accordance with current guidance.
8 Records of the safety of electrical equipment in use at the premises (PAT testing) must be on the premises and renewed annually.
9 There must be an assessment as to whether there is asbestos present in the premises and available on the premises.
10 Records of scheduled workplace / public area cleaning and the related assessments for hazardous substances in use at the premises must be available on the premises.
11 There must be records of health and safety training relating to management and employees.
12 Adequate first aid facilities will be available on the premises and adequately maintained for use.
13 A responsible person must be able to attend the premises on the request of an authorised officer within 20 minutes. The responsible person must have a good understanding of all the legal requirements in relation to the lawful operation of the business. The responsible person must have evidence that they have attended the Compliance Awareness Day.
14 On Saturday night / Sunday morning no orders for food will be taken after 04.45 hours and the premises will be closed to the public at 05.00 hours.
