Annex 2 - Conditions consistent with the Operating Schedule
1 No entertainment of an adult nature will take place on the premises.
2 The premises are to be equipped with a closed circuit television system, which is appropriate to its purpose and provided and maintained to the satisfaction of the enforcement authorities. CCTV recordings shall be retained for a period of 30 days and made available within a reasonable time upon request by the enforcement authorities.
3 The premises have installed a wall-mounted screen, no smaller than 32", mounted in a prominent position within the premises, so that patrons can view live-recorded CCTV footage.
4 At any time the premises are open to the public after 3am, the premises licence holder shall employ at least one registered door supervisor.
5 The door supervisor(s) shall wear reflective identification as approved by Lancashire Constabulary.
6 Any individual employed on the premises to carry out a security activity must be licensed by the SIA.
7 Frequent collection of all litter dropped within the vicinity of the licensed premises to be collected and removed, at the licence holder's expense, at a frequency of not less than 60 minute intervals during opening hours. This includes washing away to the gutter any spilled food.
8 The premises licence holder shall provide suitable containers for the storage of waste, which are constructed and maintained to prevent the removal of such waste by vandals, thieves, accidental spillage or inclement weather.
9 Noise from any mechanical ventilation or refrigeration plant shall be inaudible within the nearest sensitive properties or, at the discretion of the Local Authority, shall not exceed some other pre-agreed limit, which does not cause unreasonable disturbance to local residents.
10 The collection and disposal of refuse should not take place externally between the hours of 2200 - 0800.
11 The premises licence holder shall ensure that adequate ventilation is provided so as to ensure that cooking, noxious or persistent smells generated at the premises do not cause nuisance to properties within close proximity.
12 The premises licence holder shall provide suitable containers for the storage of waste, which are constructed and maintained to prevent the removal of such waste by vandals, thieves, animals, accidental spillage or inclement weather.
13 Any external light source associated with the premises shall not cause a nuisance or disturbance to any property within close proximity.
14 The premises licence holder will arrange for litter dropped in the vicinity of the licensed premises to be collected and removed at the licence holder's expense at a frequency of not more than sixty-minute intervals during opening hours.
15 Evidence of the safety and maintenance of the electrical installation for the premises must be available on site for inspection and renewed at least every 5 years.
16 Evidence of the safety of electrical equipment in use at the premises (PAT testing) must be available on site and renewed annually.
17 Evidence (provided by a CORGI registered engineer) of the safety of gas appliances in use at the premises must be available on site and renewed annually.
18 Evidence of management of asbestos containing materials (asbestos survey) relating specifically to the premises must be available on site.
19 The permitted hours for late night refreshment may be extended to 0500 hours on Bank Holidays (including Easter weekend), New Year's Eve and Christmas Eve.
