Annex 2 - Conditions consistent with the Operating Schedule
1 No entertainment of an adult nature will take place on the premises.
2 The premises are to be equipped with a CCTV system which meets the criteria detailed in the Lancashire Constabulary guidance document and which is well maintained and fully functional during the hours the premises are open to members of the public.
3 CCTV tapes shall be retained for 31 days and recordings shall be made available to the police upon request.
4 The premises shall have installed a wall mounted plasma screen, no smaller than 32", mounted in a position within the premises so that patrons can view live recorded CCTV footage.
5 A clear, legible and conspicuous notice requesting patrons to avoid causing noise, nuisance or disturbance to local residents shall be displayed at every exit.
6 Frequent collection of all litter dropped within the vicinity of the licensed premises to be collected and removed, at the licence holder's expense, at a frequency of not less than 60 minute intervals during opening hours. This includes washing away to the gutter any spilled food.
7 The premises licence holder shall provide suitable containers for the storage of waste, which are constructed and maintained to prevent the removal of such waste by vandals, thieves, animals, accidental spillage or inclement weather.
8 The licensee shall ensure that staff arriving before 0900 hours or departing late at night when the business has ceased trading conduct themselves in such a manner to avoid disturbance to nearby residents.
9 A full register of portable and movable electrical equipment and test results must be carried out annually and documented evidence available for inspection on the premises.
10 A full periodic inspection and test of the electrical installations must be carried out every five years or at intervals recommended by a competent person, documented evidence of the inspection and test must be available for inspection on the premises.
11 Documented evidence that all gas appliances on site are safe to use must be available for inspection on the premises and renewed on an annual basis.
12 An asbestos assessment relating to this premise must be available for inspection on the premises.
13 All employees must have access to adequate sanitary convenience facilities at all times they are at work.
