Annex 2 - Conditions consistent with the Operating Schedule
1 No entertainment of an adult nature will take place on the premises.
2 Alcohol shall not be sold in an open container or be consumed in the licensed premises.
3 Another member of staff, whose identity is known by all other staff, shall be nominated to act for the Designated Premises Supervisor whenever the Supervisor is absent from the premises.
4 The premises are to be equipped with a closed circuit television system which meets the criteria detailed in Lancashire Constabulary's Guidance Document CCTV/AW1, and which is well maintained and fully functional during the hours the premises are open to the public.
5 A clear, legible and conspicuous notice requesting patrons to avoid causing noise, nuisance or disturbance to local residents shall be displayed at the exit.
6 The Licence holder is to support and rigorously enforce the Challenge 21 proof of age policy.
Any person who looks or appears to be under the age of 21 shall be asked to provide identification that they are over the age of 18. The following are the only forms of identification acceptable:
i. UK photo driving licence
ii. Passport
iii. Proof of Age Standards Scheme Card
OR any other nationally or locally approved form of identification which may be introduced in the future.
If no suitable identification is provided, the sale of alcohol will be refused.
7 A refusals book will be kept at the premises.
8 All staff to have received suitable training in relation to the proof of age scheme. Records to evidence this will be made available to an authorised officer upon request.
9 A fire risk assessment to be completed by a competent person and the significant findings made available for inspection by a Fire Officer, Police Officer or authorised Officer of the Council.
10 A suitable and sufficient procedure should be established, to be followed in the event of an emergency.
11 A suitable and sufficient fire alarm system to be installed and maintained in accordance with BS 5839 or equivalent standard. Maintenance records should be made available for inspection by a Fire Officer, Police Officer or authorised Officer of the Council.
12 Suitable and sufficient means of fighting fire to be provided and maintained.
13 Staff shall receive training in action to be taken in the event of a fire or suspicion of a fire, this shall be recorded and records made available for inspection by a Fire Officer, Police Officer or authorised Officer of the Council.
