Annex 2 - Conditions consistent with the Operating Schedule
1 No entertainment of an adult nature will take place on the premises.
2 The premises are not open to the public at any time.
3 The Designated Premises Supervisor, when present, and all members of staff shall ensure that all lawful instructions and / or directions given by the Police are compiled with.
4 No alcohol / drink shall be removed from the premises in an unsealed container.
5 A sufficient CCTV system shall be installed at the premises in liason with and to the satisfaction of Lancashire Constabulary and shall be used to record during all hours that a licensable activity takes place on the premises.
6 The images recorded by the CCTV system shall be retained in unedited form for a period of not less than 31 days.
7 There shall be in place for the premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 21 years to produce a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available a photo driving licence or passport indicating that they are over 18 years of age.
8 A personal licence holder shall be on the premises at all times during which intoxicating liquor is being sold or supplied to customers, except in the case of an emergency.
9 A record of all transactions will be maintained. The transactions record will contain the name, address, details of the order along with the requested time of the order. This shall be documented and available for inspection to Police officers or other authorised persons.
10 A refusals record shall be kept at the premises, and all refusals shall be documented in the refusal book. This will include the name, address, details of the order, requested time of order and this shall be available for inspection to Police officers or other authorised persons.
11 All staff will receive complete training in age related products prior to opening a till on the sales floor. Refresher training will be completed every 6 months and will be fully documented and be available for inspection to Police officers or any other authorised persons.
12 There will be no direct sales to the public from the premises at any time.
13 There will be no direct sales of alcohol from the delivery vehicle.
14 Staff shall be trained in how to recognise and refuse service to customers who are intoxicated and how to handle potential troublemakers and defuse difficult situations.
15 Upon delivery of alcohol all customers who appear younger than 25 shall be asked to produce PASS accredited identification.
16 Delivery shall be refused to people who are known by staff to cause trouble, or for potential anti social behaviour.
17 There shall be no public access to the premises, and members of the public will not be allowed onto the premises.
18 Company policy shall be that only one person, the person who has ordered the alcohol, will be permitted to collect purchases from the van upon receipt of delivery.
19 All deliveries of alcohol shall be delivered to a premises address with a valid postcode.
20 All staff and delivery drivers will complete training in relation to personal safety and security. Training records will be kept at the premises.
21 No alcohol will be delivered to any person who the driver considers to be intoxicated.
22 Delivery drivers shall be instructed to enter and leave vehicle quietly, to park considerately, and at all times have in mind the neighbours and in particular residential occupiers.
23 The person who orders and pays for the alcohol delivery must be the same person who collects the alcohol when delivery takes place.
24 No person under 18 years of age shall be employed upon the licensed premises.
