Annex 2 - Conditions consistent with the Operating Schedule
1 No entertainment of an adult nature will take place on the premises.
2 All staff will be suitably trained to ensure all the licensing objectives are promoted on the premises.
3 CCTV will be installed internally and externally at the premises and will comply with the following:
a. The CCTV system shall be installed, maintained and operated to the reasonable satisfaction of Lancashire Constabulary. All public areas of the premises are to be covered by the system.
b. The system will display on any recording the correct time and date of the recording.
c. The system will make recordings during all hours the premises are open to the public.
d. VCR tapes or digital recording shall be held for a minimum of 31 days or 21 days respectively after the recording is made and will be made available to the Police or any authorised persons acting for a Responsible Authority upon request.
e. The system will, as a minimum, record images of the head and shoulders of all persons entering the premises.
4 A staff member who is conversant with the operation of the CCTV system will be on the premises at all times the premises are open to the public. This staff member will be able to show police recent data or footage with the absolute minimum of delay when requested.
5 A notice will be displayed at the entrance to the premises advising that CCTV is in operation.
6 All exit doors will be easily openable, all means of escape will be unobstructed, immediately available and clearly identifiable.
7 Adequate and appropriate supply of first aid equipment and materials will be available on the premises.
8 No live or recorded music is to be played on the premises.
9 Noise will not emanate from the premises such as to cause persons in the neighbourhood to be unreasonably disturbed.
10 To prevent the sale of alcohol to persons under the age of 18 years suitable proof of age from young people attempting to purchase alcohol will be required. The 'Challenge 25' policy will be incorporated and maintained on the premises.
11 All staff to have received suitable training in relation to the proof of age scheme to be applied upon the premises. Records to evidence this will be made available to an authorised officer upon request.
12 An incident book will be maintained, in which shall be recorded:
a. All incidents of crime and disorder
b. Refused sales to suspected under age / drunken persons
c. A record of any person refused admission or asked to leave the premises
d. Details of occasions upon which the Police are called to the premises.
That book shall be available for inspection by a Police Officer or authorised officer of a Responsible Authority.
13 No alcohol shall be sold in single cans.
14 At least one personal licence holder will be available (this does not mean necessarily present at the premises) at all times that alcohol is on sale.
15 The premises may only be operated as a Polish Delicatessen. Only Polish alcohol may be sold from the premises.
