Annex 2 - Conditions consistent with the Operating Schedule
1 No entertainment of an adult nature will take place on the premises.
2 All employees will be taught the four licensing objectives and will be requested to promote these where possible. The premises licence holder will work with the local authorities and implement any advice where necessary.
3 Alcohol shall not be sold or supplied on the premises otherwise than to persons taking table meals on the premises and for consumption as an ancillary to the meal and to persons waiting to be seated for a meal.
4 At least one Personal Licence holder (whose identity will be known to all other staff engaged in the sale or supply of alcohol) will be available whilst the sale or supply of alcohol is being undertaken, except in the case of emergency.
5 An authorisation of sales, signed and dated by the DPS, shall be kept at the premises showing all persons authorised by them to make sales of alcohol at the premises.
6 All staff will receive training in relation to the proof of age scheme to be applied upon the premises. Records to evidence this will be made available to an authorised officer upon request.
7 An incident book will be maintained, in which shall be recorded:
a. All incidents of crime and disorder
b. Refused sales to suspected under age / drunken persons
c. A record of any person refused admission or asked to leave the premises
d. Details of occasions upon which the Police are called to the premises
e. The use or discovery of drugs
That book shall be made available for inspection by a Police Officer or authorised person.
8 CCTV will be installed internally and externally at the premises. Said CCTV system shall comply with the following criteria:
a. The CCTV shall be installed, maintained and operated to the reasonable satisfaction of Lancashire Constabulary. All public areas of the premises, with the exception of the toilets, shall be covered by the system;
b. The system shall display on any recording the correct time and date of the recording;
c. The system shall be recording during all hours the premises is open to the public;
d. VCR tapes or digital recording shall be held for a minimum of 30 days after the recording is made and will be made available to the Police for inspection upon request;
e. The system shall, as a minimum, record images of the head and shoulders of all persons entering the premises.
9 A staff member who is conversant with the operation of the CCTV system will be on the premises at all times the premises are open to the public. This staff member will be able to show Police recent data or footage with the absolute minimum of delay when requested and will be able to provide a copy of such footage to the Police in a readable format within 24 hours.
10 The Police Licensing Unit shall be notified on any occasion when the CCTV system is to be inoperative for a period in excess of one working day and shall provide a certificate from a competent person stating the reason for the system being inoperative and the measures which have been taken to satisfy the licence conditions.
11 No bottles / glasses are permitted to be taken off the premises.
12 Staff will make regular checks of toilets. All checks to be recorded and incidents of crime and disorder noted. Records will be made available to the Police on request.
13 Music will not be played to a level where it can be heard from outside the premises.
14 Any mess created outside the premises will be cleared up.
15 A Challenge 21 policy is in operation at the premises - any person who looks under the age of 21 is requested to provide proof of age by way of a photo driving licence, passport or PASS accredited proof of age card.
